Chicago, Ill. (312) 413-1567
CFDC is a licensed provider of evaluation and intervention services in Illinois' early intervention system. This program offers state-of-the art services to the child and family that are individualized, evidence-based, and family-centered. The child's caregiver is an active participant in early intervention sessions where they learn to promote their child's growth and development. Licensed and credentialed clinical staff address the assessed needs of infants/toddlers with disabilities who have delays in motor, communication, self-care, social-emotional, feeding/swallowing, sensory integration, and/or cognitive areas. This program also offers evaluation and intervention services for children who have autism spectrum disorders, challenging behavior, and/or are medically fragile. Services are delivered in home, community and childcare settings and are billed to the family's insurance, the State, or the parent.
